The investigation uncovered critical flaws in the original design, construction, and maintenance of the building’s reinforced concrete flat plate system, particularly in the pool deck area. It highlights key engineering lessons related to punching shear failures, load redistribution, long-term sustained loading effects, and the consequences of design deficiencies and inadequate peer review.
